Health Care Privacy

Privacy of Your Records

Information about your health and care is protected by a body of federal regulations referred to as HIPAA (Health Insurance Portability and Accountability Act). HIPAA policies give you more control over who can see your private medical information. They also describe what health care providers can and can not do related to your protected health information.

Know Your Privacy Rights

We make every effort to keep you informed of your rights relative to HIPAA policies and any changes in our policy relative to the privacy of your records.

  • You are welcome to read our privacy notice in English or Spanish.
  • We keep on file a form in English or Spanish that you have signed acknowledging that you have read our privacy notice.

Knowing How Your Files Have Been Accessed

You may request a summary of accesses to your computerized medical record within the past month. This information includes the department, the division, and the name of the person who accessed your record, along with the date and time, and the function performed. To request a summary of computerized record access within the past month, contact the Office of The Patient Experience at 319-356-1802.
